Terms and Conditions

1. Services

Port Therapy, LLC (“Port Therapy,” “we,” “our,” or “us”) provides pediatric therapy services, including but not limited to:

  • Physical Therapy

  • Speech-Language Pathology

  • Feeding Therapy

  • Developmental Assessments

  • Consultations and Educational Services

  • Other pediatric rehabilitation services

All services are provided in accordance with applicable federal and state laws, licensing requirements, and professional standards.

2. Appointment Scheduling and Attendance

Patients and families are responsible for:

  • Providing accurate and current contact information.

  • Attending scheduled appointments on time.

  • Providing appropriate notice when canceling or rescheduling appointments.

Repeated missed appointments, late cancellations, or failure to comply with attendance policies may result in modification, suspension, or discharge from services.

3. Insurance and Financial Responsibility

Patients and legal guardians are responsible for:

  • Providing accurate insurance information.

  • Understanding their insurance benefits and coverage.

  • Paying all applicable copayments, deductibles, coinsurance amounts, and charges for non-covered services.

Verification of benefits is not a guarantee of payment by an insurance carrier.

Any balance not paid by insurance remains the responsibility of the patient or legal guardian.

Port Therapy reserves the right to charge fees for:

  • Missed appointments

  • Late cancellations

  • Returned checks

  • Unpaid balances

  • Completion of forms, reports, or administrative requests not covered by insurance

4. Patient Information

Patients and legal guardians agree to provide complete and accurate information regarding:

  • Medical history

  • Insurance information

  • Contact information

  • Emergency contacts

  • Consent documentation

Failure to provide accurate information may impact service delivery and insurance reimbursement.

5. Electronic Communications

Port Therapy may communicate with patients and families through:

  • Telephone calls

  • Voicemail

  • Email

  • Text messages (SMS)

  • Patient portal communications

These communications may include:

  • Appointment reminders

  • Scheduling updates

  • Billing notifications

  • Care coordination information

  • Administrative announcements

While reasonable safeguards are used, electronic communications may not always be completely secure.

Patients may opt out of non-essential communications by contacting our office.

6. Text Messaging Terms

By providing a mobile phone number, you consent to receive text messages from Port Therapy regarding:

  • Appointment reminders

  • Scheduling updates

  • Service-related communications

  • Billing notifications

Message frequency may vary.

Message and data rates may apply according to your wireless carrier plan.

You may opt out of receiving text messages at any time by replying STOP or by contacting Port Therapy directly.

Text messaging should not be used for medical emergencies.

7. Medical Emergencies

Port Therapy does not provide emergency medical services.

If you believe your child is experiencing a medical emergency, call 911 immediately or seek emergency medical care.

Voicemail, email, patient portal messages, and text messages are not monitored continuously and should never be used for urgent medical concerns.

8. Website Use

Visitors to any Port Therapy online services agree to use such services only for lawful purposes.

Users may not:

  • Attempt unauthorized access to systems or data.

  • Introduce malicious software or code.

  • Interfere with website functionality.

  • Use Port Therapy content without authorization.

Port Therapy reserves the right to restrict or terminate access to any user who violates these Terms and Conditions.

9. Intellectual Property

All logos, graphics, educational materials, forms, documents, policies, and other content provided by Port Therapy remain the property of Port Therapy unless otherwise noted.

Materials may not be copied, reproduced, distributed, or used for commercial purposes without prior written permission.

10. Privacy and Confidentiality

Port Therapy is committed to protecting patient privacy and complies with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

Information regarding the collection, use, and protection of personal information is outlined in our Privacy Policy and Notice of Privacy Practices.

11. Third-Party Services

Port Therapy may utilize third-party providers for:

  • Scheduling

  • Billing

  • Electronic medical records

  • Patient communications

  • Website hosting

  • Payment processing

Port Therapy is not responsible for interruptions, errors, or policies of third-party service providers beyond our reasonable control.

12. Limitation of Liability

To the fullest extent permitted by law, Port Therapy shall not be liable for indirect, incidental, special, or consequential damages arising from:

  • Use of Port Therapy services

  • Electronic communications

  • Service interruptions

  • Third-party service failures

Nothing in these Terms and Conditions limits rights or protections available under applicable law.

13. Modification of Terms

Port Therapy reserves the right to modify these Terms and Conditions at any time.

Updated versions become effective upon posting or distribution unless otherwise stated.

Continued use of services following any updates constitutes acceptance of the revised Terms and Conditions.

14. Governing Law

These Terms and Conditions shall be governed by and interpreted in accordance with the laws of the State of Maine.
